Does Empathetic Attitude Help in Better Out-come?

  • A 29-year-old white female was admitted with complains of increased urinary frequency, low grade temperature, flank pain for two days which was indicative of pyelonephritis.undefined

The Resident doctor informed that patient tries to manipulate the facts, the medical staff and pitting her primary physicians against consultants. She said that doctors are “played games” against her.

She has received medical care for her muscular dystrophy, progressive restrictive lung disease caused by severe disfiguring kyphoscoliosis, and chronic urinary tract infections secondary to multiple staghorn calculi colonized with Candida at a university residency practice since last few years. undefined

Resident doctors interviewed her to understand her physical problems and to understand how the consultants are playing games with her. During bedside round, she complained about her increase frequency of urination. Previously, she use to urinate at 9 a.m., noon, and at 3:00 p.m. each time she has to take help of prearranged paid assistant. At all other times, she avoids urinating even if she felt the need, because of the difficulty and embarrassment involved in asking for help in transportation to toilet.

Three weeks back, she was consulted to the infectious disease physician and he advised to stop her medication for candidal nephrolithiasis. She reluctantly complied, worried that she would become incontinent off the medicine. Indeed, this proved true. She expressed her

anger to the consultant. She was frustrated because medical caretakers had unable to understand her problems in day to day activities. She believes that the urologist prescribed her experimental medicines and he didn’t explained me, the drug – drug interaction with contraceptive pills. And thereafter my menses was also disturbed. She expressed anger that the consultant physicians neglected her “identity as a woman.” She was also angered that her resident physician was frequently unaware of the changes in plans made by these consultants.
